What is PerformCare?
*PerformCare is an independent New Jersey state program. Creating Change, LLC is a contracted provider of their services.*
Since 2009, PerformCare has been helping New Jersey’s families and young people access publicly funded services for youth up to age 21 through the statewide New Jersey Children’s System of Care (CSOC). Help is available for children, adolescents, and young adults seeking behavioral health, intellectual/developmental disability, or substance use treatment services.
Partners of PerformCare are committed to providing FREE, voluntary services in a family driven, community-based environment that builds on the family's unique strengths, values, voice, and resources.
PerformCare and its services place an emphasis on the family or caregiver as playing a central role in the health and well-being of children. PerformCare involves families throughout the planning and any service delivery process in order to create a system that values and promotes the advice and recommendations of the family, a system that is friendly to families and one which provides them the tools and support needed to create successful life experiences for their children.
PerformCare associates are available 365 days a year to connect eligible children to individualized care.

How To Begin
To start in-community therapeutic services, prior authorization through PerformCare is required. A parent/guardian must make the call to PerformCare for more information on accessing these services. (1-877-652-7624)
